§ 98-37. Applications.

  • All applications for permits for the operation of vehicles for hire on the streets of the city-parish shall be made to customer service, and shall set forth:

    (1)

    A description of the current chauffeur's license issued to any driver of a vehicle for hire to be operated under the permit, once obtained.

    (2)

    The name and address of the applicant, who shall be the owner or lessee of such vehicle(s) for hire or an association. If the applicant is a corporation, partnership, company, association or other legal entity of any kind or character, the application shall state the names and addresses of all owners, partners, officers, directors, members and/or management.

    (3)

    If the applicant for any permit hereunder is not an individual, all owners, partners, directors, officers, members and management of the entity must possess the same qualifications required of any individual applicant. All such qualifications will be shown by an affidavit submitted by each individual along with the application.

    (4)

    The trade name, if any, under which the applicant does or proposes to do business.

    (5)

    The class, type, seating capacity, make, model, vehicle identification number, design and color scheme of each and every vehicle for hire, and any lettering and marks to be used thereon.

    (6)

    A copy of the title of the vehicle for hire, if owned by the applicant.

    (7)

    Whether the applicant, its owners, partners, directors, officers, members, and/or management has been convicted of a felony under the laws of the United States, State of Louisiana or any other state or country within seven years immediately preceding the date of filing for an application for a permit. If answered in the affirmative:

    a.

    The crime for which convicted;

    b.

    The date of conviction;

    c.

    The court in which convicted;

    d.

    The sentence and, if time was served in prison, jail or any other reformatory institution, the period of time served and the location of the facility at which the time was served; and

    e.

    Whether the person is currently on probation.

    (8)

    Whether the applicant has or intends to locate his place of business, stand and/or dispatch office within the Parish of Lafayette, or if said business or individual intends to operate within the city-parish but will have its place of business, stand and/or dispatch office located outside the Parish of Lafayette.

    (9)

    Whether there are or were claims and/or judgments against the applicant for damages alleged to have resulted from the negligent operation of a vehicle for hire within the five years immediately preceding the date of filing an application for the permit, and if so, the disposition or current status of the claims and/or judgments.

    (10)

    The ability of the applicant, or if an association, of each member of said association, to adjust claims and/or pay judgments in the event of injury and/or damage to person or property by reason of the negligent operation of the vehicle for hire.

    (11)

    A statement of the facts showing a demand for the service proposed to be rendered by the applicant, and the experience of the applicant in rendering such service prior to the application.

    (12)

    Any other information which may be required by customer service.

    (13)

    The application may be made upon printed forms to be prepared under the direction of and available at customer service.

    (14)

    Proof satisfactory to customer service that all necessary insurance has been secured with reference to each vehicle for hire for which a permit is sought.
