§ 78-212. Application.

  • (a)

    Any person desiring a permit required by this subdivision shall make application to the planning, zoning and codes department. Application for such permit shall be made to the planning, zoning and codes department on forms prescribed by the department. The permit shall be approved as to location and design, as well as pipe size and elevation and quality of pipe, by the public works department. Each application shall be accompanied by a plat or sketch, which shall be part of the application for a permit.

    (b)

    The applicant shall make application at least ten days prior to the date his contemplated work is to begin. The application shall be in the form of an affidavit stating the full name and address of the applicant, the location of his principal office and place of business, the name and address of the officers if the applicant is a corporation, and the partnership name and the names and addresses of all parties if the applicant is a partnership. Before the permit shall be issued, the application must be accompanied by:

    (1)

    A certified copy of the Charter if the applicant is a corporation incorporated under the laws of the state.

    (2)

    A certified copy of its permit to do business in the state if the applicant is a corporation incorporated under the laws of some other state.

    (3)

    A plat showing the location and specifications for the construction of the excavation, which plat shall include:

    a.

    A survey of the proposed location of the line. No survey is required for driveway culverts.

    b.

    The names of the property owners of the adjoining property.

    c.

    Specifications showing the depth at which the line will be laid.

    (4)

    A certificate of compliance from the public works department stating that specifications are such that the line will not interfere with the maintenance of the road.

    (5)

    A certified copy of the instrument by which the city-parish consolidated government acquired its servitude or fee interest.
